1. What are the office hours?Our office is open from 8:30 AM to 5:00 PM. Our physicians are available Mondays to Thursdays. Each physician has different days and hours available to patients. Please call our staff at 219-769-0054 to discuss the physician hours of availability.
2. What do I need to bring to my appointment?
Driver's License, State ID, Insurance Card, current list of medications, and office co-pay. Please bring a referral from your PCP if required by your insurance plan.
3. Does your doctor accept my insurance?
Our office does accept many insurance plans. Please call the telephone number on the back of the insurance card to find out if our physicians are in network. Some insurance plans may require a referral from your primary care physician. Our providers information are listed to assisted you.

3. How much is my co-pay?
Insurances may charge a co-pay, co-insurance, deductible and/or out of pocket. Please refer to your insurance company for specialist office visit co-pay. All co-pays are to be collected at the time of the appointment with physicians.
4. Do you have oxygen on site for patients that need it?
Yes, our office has oxygen on site to accommodate any oxygen-dependent patients.
5. Can I have a sleep study if I have a referral for the test?
We recommend all patients see our physician prior to having sleep studies. Our physicians complete a medical exam based on all insurance requirements. This allows our billing department to obtain any authorizations required for the sleep apnea test. If the sleep study shows sleep apnea, there is a second test to determine the treatment for sleep apnea. After the treatment is determined for sleep apnea our physicians will write the order for the equipment. Each patient will meet with a respiratory therapist to learn how to use the equipment, change the supplies and clean it. Each patient must use the equipment for six to eight weeks and follow up with the physician to determine therapy usage and success. This is a requirement from the insurance company to help continue to pay for the therapy. The equipment is rented by the insurance company to ensure success for the patient. Our staff closely monitors this process with the patient and physicians. Our monitoring system does not only assist insurance companies. It gives us easy access to assist DOT and CDL drivers with their monitoring to maintain federal regulations.
6. Do you suggest oral appliance?
Oral appliances have very specific requirements for sleep apnea patients. We do not provide oral appliances in the office. Referrals may be arranged if patients meet the requirement of their insurance plan. Some insurance do not cover oral appliances. Please refer to your insurance plan.
7. How much will a sleep study cost?
Please refer to your insurance company for benefits. You may refer to the information below.
• Diagnostic Sleep Study in the doctor's office. Procedure code is 95810 with a diagnoses code of G47.33.
• Titration Sleep Study in the doctor's office. Procedure code is 95811 with a diagnoses code of G47.33

8. Do you test with home sleep studies?
Home sleep studies are available. The insurance will determine which testing is necessary for you. Some health conditions are too severe for home sleep testing and will require in lab sleep test. This will be determined by the physician and insurance plan.
9. Why are there two sleep studies?
The diagnostic sleep study is to find out if you have sleep apnea. The titration sleep study helps determine treatment for sleep apnea. Depending on the severity of the sleep apnea and the patient's condition more than one titration may be needed to determine the right type of equipment for you.
10. Do you take orders for patients that have a pap device already?
Our physicians are happy to assist patients who have been diagnosed with sleep apnea. Our sleep coordinator will assess the needs of the insurance policy prior to writing orders for patients. Insurance policies have specific contracts with facilities to maintain orders for patients. Our office does offer cash pricing for the uninsured for pap devices and supplies. 11. What do I need to know for my sleep study?
• Our staff arrives at 7:45 PM for sleep study patients and leaves promptly at 6:00 AM
• Sleep Studies traditionally run seven nights a week to accommodate all patients and their families.
• Please bring ID to sleep lab, your insurance will be verified prior to testing.
• Please shower before you come to your test.
• Please do not add any oils, powders or lotions to the skin. You may apply deodorant
• Please be aware technicians will apply glue to multiple areas of the hairline to secure leads to monitor the different stages of sleep during the test. We do not recommend any hair pieces during this process example toupees, hair extensions, or weaves. We understand the cost it takes replace and apply these items. WE CAN ACCOMMODATE TEST PRIOR TO HAIR APPOINTMENTS. Sleep Studies traditionally run seven nights a week to accommodate all patients.
• Please trim or shave any facial hair to accommodate facial leads places during the test. This will monitor jaw movements during the test. Our technicians understand patients that do not remove facial hair. They will do their best accommodate the patients' needs.
• Please wear loose fit clothing during the test. Pajamas that button down of the top are preferred for the heart monitors that will be applied on the chest. Shorts or pajama bottoms work best to accommodate leg leads to monitor leg movements.
• Please bring any medications with you that you may take at night. You may take them prior to coming for the test is that accommodates your needs. Our staff my store any medication in a medical refrigerator for you if needed.
• Please bring any essentials you need to shower in the morning if needed. Our office will provide towels, washcloths and hand towels if needed.
• Do not drink caffeine after Noon the day of the test.
• Please let the staff know if there are any special accommodations needed for the sleep study.
12. When will my test results be ready?
Test results may take up to 14 days to interpret. Our physicians review all of the data from your sleep study to ensure a proper diagnosis. The sleep coordinator will call you with your test results as soon as they are ready. You may call the office at 219-769-0054 to check the status of the reading.
